Waiver of Liability

I hereby waive and release Miller Mulligans from any and all liability for any injury or illness incurred throughout the duration of my participation, usage of the building, golf simulators, and related equipment.

I recognize the physical exertion involved in the use of the building, golf simulators, and related equipment, and attest and certify that I/my child am/is physically fit to participate safely, and I have not been advised otherwise by a healthcare professional.

I expressly agree to release and discharge Miller Mulligans and all affiliates, employees, representatives, successors or assigns, from any and all claims or causes of action.

This waiver and release of liabilities, without limitation, covers all injuries to me/my child which may occur. Regardless of negligence, as a result of (a) my/my child's use of any and all Miller Mulligans equipment, products and amenities, (b) the sudden unforeseen malfunctioning of any equipment, and (c) me/my child slipping and/or falling while in Miller Mulligans, or on Miller Mulligans premises, including adjacent sidewalks and parking areas.

I will be personally liable for any and all damage to the building, golf simulators and related equipment caused by my careless, reckless or intentional actions.

I acknowledge that I have carefully read this waiver and release and fully understand that’s a release of liability.

I agree to voluntarily give up the right that I may otherwise have to bring a legal action against Miller Mulligans for negligence, or any other personal injury or property damage of loss action.
